The food is deplorable, the facility is in a time warp, the ATTITUDES & BEDSIDE MANNER from the majority of the nurses is indescribable. And I will just let the photos speak to the 'upkeep and cleanliness'. Unfortunately, the videos are too short so they cannot be uploaded, but thankfully I took photos as well. This review is from April 2026!! I wish I could give this place -10 stars. With the exception of 5-6 staff members this place needs a COMPLETE OVERHAUL!! The food is deplorable, the facility is in a time warp, the ATTITUDES & BEDSIDE MANNER from the majority of the nurses is indescribable. And I will just let the photos speak to the 'upkeep and cleanliness'. Unfortunately, the videos are too short so they cannot be uploaded, but thankfully I took photos as well. For context, the photo with the soaking wet sheets/caution sign, that was outside of my mom's door from around 4am Sat (from a busted pipe) to almost 2am Sunday when I left. the water bugs are from various times in ONE night. The one stuck in the hole crawling its way back out was one that I thought I killed on FRIDAY night only to have it crawl its way back out on Saturday night as I'm talking to the maintenance guy who was trying to convince me that there was no water bug there. I got her out of that room and out of the building ASAPTUALLY!! There is so much more to say, but I'll stop here or I will never stop typing. If you LOVE your LOVEd one(s), I BEG you to NOT place them in this disgusting facility!! I would love to know what facility the so-called 5 star ratings are reviewing!
My experience with the staff on 2W has been mostly pleasant. Unfortunately, there always seem to be a staff member that makes the experience undesirable. In saying this, I have to point out Ms. Sirlina Rudolphus' excellent attitude, her attention to details that helps dismiss the unpleasant experience. She is caring, friendly and is proactive in her job. Her professionalism is commendable. Staff should learn from Sirlina the importance of how to care and treat patients with respect as well as the family members. She is an asset to Crown Heights. She is caring, friendly and is proactive in her job. Her professionalism is commendable. In addition, Ms. Heather Mavangira is also as attentive and caring as Sirlina. She has been a positive experience and helpful with our concerns for our father. I also commend her professionalism. Please continue to do good for for those who depend on your care, for my kind father who is currently struggling with his chronic ailments and who has served this country as a Korean veteran. Thank you for what you do.
My mother MS. Brown has been a patient here for, Few months now and I really appreciate the care and effort the CNA has put in with her. I chose here because of Ms Sirlina Sw she has Been a great help. All the Sw are awesome Mr Brown Thank you .

As a dialysis patient it was important to me to find a rehab center that had both rehabilitation and dialysis I do not regret choosing this center The staff is very accommodating the doctor is on top of my condition Should the situation arise again I will not hesitate choosing crown heights .

Went there for the fist time to see my brother whom I haven’t seen in years and staff seemed nice, the place was clean and seen lots of smiles including my brother’s. Though I was only there for 5 mins because it was almost time for me to take the bus back home, a 4 hr trip, I was very happy to have seen my big brother and plan to go back soon God willing. God bless my brother and this facility. See you again soon my dear brother 🙏🏽❤️
The nurses and physicians give excellent care to my mother. The residents are involved in many activities that keep the active and content.
I would like to THANK CROWN HEIGHTS STAFF ( ESPECIALLY NURSE MS M DAVIS , SHE WAS NOT ONLY A NURSE TO MOMMA LOIS BLAKELY MS. DAVIS TOOK. SOMETIME WITH MOMMA AND MOMMA LOVED HER AS A DAUGHTER MS. AND FEW OTHERS MS BENSON MS OROSCO WITH THE SOCAIL SERVICE WOKERS DEPT. THE BLAKELY FAMILY APPRECIATE YOU GODBLESSU AND GODBLESSFAMILY IN JESUS MIGHTY NAME AMEN 🙏🏾
