I was told i have Phonophobia by a mental hygiene counselor at Martine Center Nursing and Rehabilitation. I was told i needed a private room by the director of Martine Center Nursing and Rehabilitation and that i would have to be transferred if they din't have one. I was transferred and and given a transfer agreement. When i got here that agreement was demanded of me, and i gave it out of fear. This facility didn't do a re assessment as the law required because i was new resident. After about 5 weeks of living thru an unrelenting horror i became suspicious. I asked for a copy of the transfer agreement and my ombudsman made sure i received it. I read that the sending nursing home left out the 2 reasons i was transferred. it said i had unspecified personality disorder, and the need for a private room was left out. The last thing a Marine employee said to me was "you can't call this nursing home anymore. Martine was involved in the 84 million dollar settlement that the attorney genreral brought against centers health care the corporation that owns both facilities. The intent of this review is not to point fingers and say how bad you are, it is just to reveal the facts of my stay. I was told by my ombudy alp's medicaid funded for me. i hope that goes thru. I was admitted for unsafe housing provided by Pibly Residential Programs Inc. supportive housing opportunities program. I am looking forward to more independence. I don't care for the restriction here. I read that the senior justice law firm wrote words to the effect, the only way to get your rights met in a nursing home is with a law firm..

Google said to write your experience in your review of a company. That enables me to cite my most recent experience with this company. My brother had talked to the director of social work recently and he told me they are going to get me housing in 2 or 3 months. That is great for me. I was in unstable housing and that is what I need. I have medical concerns that hopefully can be met during the waiting period. That is something good.

I have Phonophobia. It has been ignored that I do. It is reaction to noise that poses a danger to me because I am unable to control my self. So I leave the area where it is happening. Even if I don't get what I came for like to emergency room because I can't tolerate it. The answer is to get a letter of medical necessity from my doctor. I was told my doctor would see me and she never came. I am speaking up because I believe that people are going to help me with this. Whether they do or not I have tried. There is always the law. I was transferred here to get a private room. When I arrived a man demanded my copy of the transfer agreement. I gave it to him. If I didn't I didn't know what would happen. I was not provided a private room but a shared room with two other people. I was told that I would be seen by a doctor so I can get a letter of medical necessity for the phonophobia. She never came to see me.

Awesome staff

It was a blessing because I'm an army veteran who battled with mental illness for years I got out of the military 2013 and I did not get heels until 2025 now I've been a song about it called. God saved me part one when I came to the hospital administered my single to the Veterans there and they were crying saying that I brought back memories now I decided I wanna visit there every Friday and Minister my songs to them. I am a Christian artist so my mission is to spread the gospel all over the Earth so how people be here just like I was here and give their life to Jesus Jesus Christ so they will never be sick again I pray that the name of Jesus every patient in the rehab is healed and also pray that each time that Minister they will receive blessings in your life and increase in protection. God can save anybody from any sickness, no matter how big or small it is and my people get healed all the Time things like AIDS STDs, and many more they have all been cared in. Jesus is the only one that can perform miracles like that so I believe if I keep ministering every Friday with Elder John, who is also from my church Christ Embassy I know for sure that those people in there will be healed and delivered from whatever evil is living side of them when administered yesterday I had all my own emotions because I never ministered in a rehab before. no, that I did. It made me realize I should be doing this every Friday so that is what I wanna do since I'm in college but do not have classes on Fridays. I will always come with Elder John so we can spread the gospel of Jesus Christ to the second loss who have no one to support them in the name of Jesus how support everyone that I come in contact with any rehab they receive increased grace increase protection increase on every side. I pray that whatever they manifest that is in the will of God they will get it also pray that everybody was in there with their workers or patients or whatever you wanna call the people there. Do not go to hell in Jesus name since I will be coming there everybody will be prepared for the return of Jesus Christ because whenever he comes, the Earth will be called up into the sky meaning Jesus is in the sky, and he appears all of the people that he chose will be rapture and meet him in the sky, but the wicked will be left behind, so I definitely do not want any of the veterans to go to hell or the workers so as much as a man there has as much answers to everybody's prayers that is in the rehab, including mine I find it funny The elevator is not working when I was trying to leave, and I made a joke to the nurse that I believe God👑 didn't want me to leave at that moment which just made me use my time to speak in tongues, which is the angelic language. God created that only he can understand. God Bless you all and God will be helping all of you whether worker or night

To Whom It May Concern, I am writing this letter to formally express my deep concern and dissatisfaction regarding the care my mother received at your rehabilitation facility. Based on our experience, I give this facility a rating of 0 out of 5 stars. From the moment my mother arrived, the level of care was unacceptable. Her bed was not functioning properly and had no safety rails, creating a serious fall risk. Her personal belongings repeatedly went missing, and staff showed little concern or accountability. My mother, who is vision impaired, was unable to sit up, eat properly, or take her prescribed medication for several days. Staff failed to provide a safe or effective alternative method for administering her medication despite suggestions being made. As a direct result of this neglect, her blood pressure rose to dangerous levels. After days of inaction, staff ultimately called 911 because they were unable to manage her condition—an emergency that could have been prevented with proper care. During her stay, my mother received physical therapy only once, which is completely unacceptable for a rehabilitation facility. By the second day, it was clear she was not receiving the care she needed, and I had to repeatedly question staff about transferring her elsewhere. The facility itself is unsanitary and poorly maintained. The building had a strong, unpleasant odor, and the environment felt unsafe. The food provided was inadequate, with no clear indication of what residents were being served. At night, the hallways were chaotic, with excessive noise and activity. Most concerning, residents were not being properly washed or changed. I personally witnessed this and had to come in daily to thoroughly clean my mother myself. No family should have to do this in a facility that is entrusted with the care of vulnerable individuals. While there may be one or two staff members who genuinely care, the overall standard of care reflects neglect rather than rehabilitation. Families trust facilities like yours with their loved ones’ lives, and that trust was severely broken in this case. Based on sanitation issues, lack of medical support, poor communication, and inadequate patient care, I cannot recommend this facility to anyone. I strongly urge management and oversight authorities to investigate these conditions to prevent other families from experiencing what we did. Please Do not send your loved ones here ever!!!! Sincerely, Concerned Family Member

This facility did such a phenomenal job taking care of my aunt. Every need was met and staff was extremely caring. Everyone starting with nursing to therapist work well together and showed professionalism the entire duration of my aunts stay. Thank you again for nurturing and giving her a great experience.
