If you want your parents to die faster...this is the place to go. My mom was in there for 3 months...being discharged tomorrow and is leaving worse than she went in. Most nurses there are useless...they should not even work with people...let alone the elderly. The nurse director, Rory, was very nice to my mom and myself. The CNAS are the ones that do the actual WORK. They deserve 5 stars. They are very compassionate and very nice. They are the ones that say mom is not fit to be home...they work with the patients everyday and night. They are understaffed, overworked...and UNAPPRECIATED. And to…
If you want your parents to die faster...this is the place to go. My mom was in there for 3 months...being discharged tomorrow and is leaving worse than she went in. Most nurses there are useless...they should not even work with people...let alone the elderly. The nurse director, Rory, was very nice to my mom and myself. The CNAS are the ones that do the actual WORK. They deserve 5 stars. They are very compassionate and very nice. They are the ones that say mom is not fit to be home...they work with the patients everyday and night. They are understaffed, overworked...and UNAPPRECIATED. And to make matters worse, the location changed owners a little over a month ago. The medical staff are not even giving minimal effort. They falsely document patient progress while the CNAs have zero input. While it is understood that almost all rehab centers and nursing homes have substandard care, this place seems be to trying to set the lowest goal possible. We witnessed people sitting in pools of their own urine, spoken to as if they were children and ignored when they genuinely needed help. On more than one occassion we had to rush there because mom needed help and no one came to help her. Then when we finally were there and helped her out, the staff conveniently show up, asking what we were doing, with attitude and zero actual concern. Definitely do not recommend allowing any family member recover there. You'd more likely survive solitary confinement than Silvercrest. This place is a death sentence for whoever ends up there.

My name is Raoutie Samuel, daughter of Omawattie. From the end of July 2025 through the end of Dec 2025, my mother was a resident. She was admitted with the hope of regaining her strength and recovering. Instead, I watched her health steadily decline during those five months. During her stay, she developed pressure sores, including a pressure injury to the back of her left heel. After I brought my concerns about her heel pressure injury to the attention of the wound care nurse, Renata, she changed my mother's offloading boots to huge long cushion boots up to her knee. However, she did not want the pillow placed under my mother's calves to elevate her calf and relieve pressure from her heels, despite this being an important pressure-relief measure. I only brought the pillow in after the heel issue was discovered at the hospital. She noted things incorrectly in my mom chart to suit her narrative, stating the pillow was placed under her knee, I don’t work at the rehab, the aids are the ones who have to perform proper repositioning and the dr noted to elevate calf with stack of blankets and this was not implemented and I was told by a doctor a pillow was fine. She stated that the pillow didn’t allow the aids to reposition proper. Hypocrisy. You remove the pillow under the calf to reposition and place it back. That is proper procedure. She wanted to blame me for a bed soar. They don’t reposition or clean them in that damn facility. Had she left her with the small boot and elevate her calf this would not have happen. I even mention I didn’t think her feet was breathing. The people at that facility working is careless. Approx. a week and a half after the new boots were introduced, the hospital documented bruising on her right foot that was not present on 12/23 and she was admitted on 12/28. Over time, the bruising progressed into an open wound that required ongoing treatment after she returned home. After my mother came home, I used the same pillow under her calves to keep pressure off her heels while continuing her wound care. With proper pressure relief and ongoing treatment, her left heel healed. Her right toenail also came off while she was a resident, and despite my asking, I never received an explanation for how it happened. My mother was often left sitting in her wheelchair for six to seven hours at a time without being repositioned. I repeatedly expressed concerns about the risk of pressure injuries and the need for proper repositioning, but I did not feel my concerns were addressed. She suffered recurrent urinary tract infections and was hospitalized repeatedly. With each hospitalization, I watched her become weaker as her overall health and physical condition continued to decline. What should have been a rehabilitation stay became a cycle of infections, hospitalizations, wounds, and continued decline. Throughout her stay, I made report after report, documented my mother's condition with photographs, and repeatedly brought my concerns to the attention of staff. Despite my efforts to advocate for her, I did not feel my concerns were taken seriously or addressed. Although my mother already required wound care before entering Silvercrest, she returned home with other wounds that required further treatment and ongoing medical care. Based on my family's experience, I believe Silvercrest should receive a thorough inspection. My mother's experience raised serious concerns about resident safety, pressure injury prevention, infection prevention, responsiveness to family concerns, and the overall quality of care.I hope our experience encourages a thorough review so that other residents and their families do not have to endure what ours did.Dr Bains and the wound care center is the only amazing thing about this place.The management doesn’t care one bit because I made report after report, the last straw was the bruise she developed like a week before she was supposed to come home and was discovered at the hospital.There are some good nurses.Social worker, managers, supervisors didn't care to help or do their jobs.”A Nightmare”
Nurse Jenica was a pleasure to work with. I did not like the intake lady who had the nerve to tell me she never heard of someone getting burned by an electric stem machine... The proper question she should have asked first was how did this happen then maybe she could understand or better yet keep her personal commitments to herself.
Let me start with saying not all of the staff are bad, but a majority of them don't care.I went to visit my uncle and he was gasping for air as I walked in his oxygen tube had Somehow disconnected and no one paid attention to the call bell there was that he constantly rings the bell? And they don't have time for him. Try to speak to the social worker and was told. If we don't like it, we could move him out. That's not the care My family signed up for The bosses keep rejecting my calls. I've even seen certain people thrown out of the building for expressing their concerns. About their loved ones this place is out of control and should be investigated before the cause , more harm to anyone else
This Place is horrible if you love your family member. Do not bring them here, they present themselves to care. But in actuality, they only care about the money they're under new ownership. And the new administration Is abusing their power residents that were able to go out to the gazebo of being told they can't residents that got a custom to living certain ways are being told everything's going to change. If you choose to talk back and try to defend yourself they're stating that you are not complying and will make your life more miserable. I know of one resident that was forced to leave and go into the shelter system because he was told he could not have visits with his family members or go to any of his appointments. Worst place ever, the food is bad. The staff is bad. The administration is out of control. Please don't get stuck like the residents I see here suffering.

I was there visiting a friend, the unit, room was clean The accessibility works for me. Also my friend is well taken care of, and thats what's important to me.
If you have a loved one at Silvercrest Nursing and Rehabilitation Center, please check on them frequently or consider other options. We were forced to remove our family member due to what we experienced as serious negligence. Last night, a nurse who identified herself as Claudia repeatedly refused to call emergency services, even after being told by NY Presbyterian transfer services that it was necessary. This was not a one-time issue—it happened multiple times. On 4/18/26, my aunt attempted to document what appeared to be bruising or a wound on my grandfather’s backside, and she was told to leave the room. That alone raises serious concerns about transparency and care. No family should have to fight this hard just to make sure their loved one is treated properly. Please stay vigilant.
