If zero stars was an option, I would rate zero. This review may be a few years late, but I feel it’s necessary to warn everyone to stay far away from here. My grandmother was at this rehab a few years back. Long story short, she called my mother and I complaining of chest pain. She said that she told the staff and she was ignored and pushed back into bed. When my mother and I arrived, we saw the staff physically push her into bed. If zero stars was an option, I would rate zero. This review may be a few years late, but I feel it’s necessary to warn everyone to stay far away from here. My grandmother was at this rehab a few years back. Long story short, she called my mother and I complaining of chest pain. She said that she told the staff and she was ignored and pushed back into bed. When my mother and I arrived, we saw the staff physically push her into bed. We ended up calling an ambulance because of her chest pain and other symptoms. At the hospital, they determined she had been having a heart attack. The staff at this facility not only ignored my grandmother’s concerns, but also physically restrained her while she was actively having a heart attack. Run far away from this place and do not bring your loved ones here.
My mother is currently a resident here, and our family couldn’t be happier with the care she receives. The rehab team is excellent and truly helped her regain strength and confidence. The nurses and CNAs are incredibly kind, patient. The social work department is attentive and always supportive, and administration is approachable and always available when needed. It gives us great peace of mind knowing my mother is in such caring hands. Highly recommend this facility.

After making a ton of complaints to the administrators Frank(director), Leslieanne (nursing director) and Samantha (director of Social none of your concerns/major issues get resolved. The floor staff is rude to the residents and their advocates. They lack compassion, care and respect. After making a ton of complaints to the administrators Frank(director), Leslieanne (nursing director) and Samantha (director of Social none of your concerns/major issues get resolved. The administrators lie and string you along and once you demand accountability they threaten you and your loved one. Their answer to everything is that they will conduct an investigation which is never done. If you don’t have the time to spend at the facility daily to ensure your loved one is getting the bare minimum treatment I advise you to find another facility. The health shakes and extra sandwiches your loved one is supposed to get for whatever reason comes up from the kitchen but will remain on the staff desk for hours and sometimes they fail to give them out. They will have your loved one waiting for hours in a saturated diaper which causes breakdown of the skin, if your loved one rings the bell for assistance they will not get help for hours. When I brought this to Franks attention he stated leaving a elderly person in a saturated diaper for 6 hours wasn’t a problem. If your loved one is put on the toilet they will leave them on the toilet until they feel like getting them off which can take 1 hour or more or until your loved one crawls out of the bathroom into the hallway on their own. They will leave your loved one without a shower and in the same clothing for over a week. Just recently I went to be with my loved one and there was a random pill on the bed. I got the nurse to ask her about it she stated she didn’t know about it. She took the pill and left the room. My loved one had no idea how it got there. I told the administration because I wasn’t sure if it was part of his daily medication regimen or if it belonged to someone else. I was told a investigation would take place and still haven’t heard anything. I have been requesting for my loved one to receive a proper shower for over 3 weeks and washing his hair with the shampoo I provided due to dandruff and to date it has not been done. The first floor is nice and clean once you get to the second and third floor it’s extremely unsanitary. I really should be able to give a rating of -10000000. My loved one clothing was missing for almost 2 weeks due to a room change. The administration Frank and Leslieanne kept stringing me along every day with lies. It took for me to investigate and get the director of housekeeping to take me to the old room and that’s where the clothing was found. The staff never completed packing his things to send them to the new room causing him to have to wear someone else’s clothing for almost 2 weeks. The picture is of the pill that was found on top of his foot rest on his bed. I could go on for days about this place. If it wasn’t the closest place to me and if I was able to travel I would have been moved my love one out of here!!!
The staff here are truly caring and attentive. My grandmother is currently a resident on the rehab floor. A special thank you to Samantha, the Director of Social Work, for her compassion and dedication. She goes above and beyond to support families and residents!
I can say enough about the excellent care and attention the staff has given my grandmother. Thank you for being excellent and helping give me peace of mind when we're not there.
This place is a house of horrors. My sister was there and was not taken care of - very few nurses and aides were good, most of the staff was not. The doctors at this place cannot be real doctors. I asked a question to the doctor and she said "who knows?" My sister had a rash under her breast and they treated it with cream or powder. It was cancer! And the hospital said she had this a long time. Also, she had a seizure and was rushed to the hospital. At hospital, she was checked for seizure medicine and there was no trace in her. She was supposed to be on it all the time. Also, she was in so much pain and screaming in pain and they did not try to help her with the pain. She was on Tylenol?! She was eventually placed in Comfort End of Life and she was still screaming in pain. This is what they call Comfort?! Also one of the staff that came in her room told the family your sister asked for ice-cream and she said I left to see other patients when I came back she passed. I should have gotten her ice-cream. Why would you tell us this?! So many times my sister did not have her own clothes and so many times I saw mice/rat feces on the floor and on my sister's shoes. This place should be shut down and some staff should be locked up. They should have taken much better care she might not be in the cemetery.
Hello, I am leaving a review to make mention of the great, helpful, competent, reliable, and outstanding service of the Director of Social Worker Samantha Di’Clemente. At every turn she is willing to look into and resolve any issue that arises. This is greatly appreciated!
If I could give zero stars I would. If your loved one is here for a LONG TERM stay for a nursing home I would run far away! The facility is dirty, and the staff is overwhelmed. My poor aunt was not consistently given medication, was left to rot on the floor on dirty mats, and we would often see rat/ mice droppings. Not to mention her brand new clothes would go missing frequently! The doctors here are extremely negligent, and have very few experienced staff. Please stay far away from this place!!
