Not a very respectable place to consider safe, well kept and managed. A beautiful facade in the lobby... decorated with opulent marble, a piano and polished floors and walls. Past the glass doors and this facility looks like a cross between The Shining & One Flew Over The CooCoo's Nest. Our family member and another patient stated staff are overly physically aggressive. If you're looking for a more responsible facility, do your homework. Medicaid has their website with listed facilities that are rated. Very disappointed with Eleanor and we're looking to get our family member outta this place.…

Dehydration in Nursing Homes • Not enough fluids offered – staff may not provide regular water or may ignore when residents ask. • Dependence on staff – some residents can’t get drinks themselves and rely on staff assistance. • Fear of incontinence – residents may avoid drinking to prevent accidents if bathroom help is slow. • Medical conditions/medications – diuretics, dementia, swallowing problems, or mobility issues can reduce fluid intake. • Neglect/lack of monitoring – staff may fail to track hydration status or signs of dehydration. ⸻ Urinary Tract Infections (UTIs) • Poor hygiene – not enough help with bathing or perineal care. • Infrequent toileting – holding urine too long increases infection risk. • Dehydration – less urine output means bacteria aren’t flushed out. • Catheter use – indwelling catheters raise UTI risk if not properly managed. • Weakened immune system – elderly are more susceptible to infections. ⸻ E. coli Infections • Contaminated hygiene practices – improper cleaning after toileting or poor handwashing by staff. • Food handling issues – if food isn’t stored, cooked, or served safely. • Cross-contamination – shared bathrooms, linens, or staff not changing gloves between residents. • Low immunity – older adults are more vulnerable to bacteria

The staff put in their utmost dedication to care for my Mom. We are ever grateful and would recommend this place.

When I came to The Eleanor 3/2025 I couldn't even walk. The place looks amazing as they remodeled their rehab unit . The staff there was great from the nurses to Therapist dept. everyone were great !!. I was able to get better and go home. I was Greeted and treated with respect and dignity.

This is the most horrible place . They dont take care of the people there. They let our family member Starve to death. He has pressure wounds so much worse than he had when going to this place. they dont turn them enough or at all. Asking for ice water . and being told they dont have it. How this place is even open. is beyond us. The nurses and a few aids we had run in with. are such rude people. Find another job. That you would actually enjoy. its all about the money with this dump. They refused our family member Hospice Care. Because they wouldn't get paid by the insurance. State needs to investigate this place. Sad when there own employee. Tell you about stuff going on there. DISGUSTING...
