I wouldn’t recommend Swan Lake Nursing and Rehabilitation Center to anyone. The doctor doesn’t listen. The staff doesn’t listen. I repeatedly told nurses something wasn’t right with my body — and I almost died. I went into septic shock, had to be rushed to the hospital, and ended up in the ICU. That should never happen when someone is already asking for help. There have been wounds where bandages weren’t changed for days. Call bells go unanswered — sometimes 30 minutes to over an hour. The doctor comes in and out and doesn’t want to hear what you’re actually experiencing. The nursing staff and supervision staff have lied to me more than once. Also staff members have thought I was vaping the supervisor came in I gave permission to search my room they obviously didn’t find anything and the manager told me that an RN would be in to strip search me that was totally wrong I asked to see proof in writing that they were allowed to do such a thing so end result no one ever came back… I’ve been here almost six months, and I am deeply unhappy and disappointed. This place has made an already difficult recovery even harder. Patients deserve to be heard, cared for, and taken seriously — and that hasn’t happened here. Not to mention to date they were going to send me to the hospital on a psychiatric hold when I was being antagonized by a resident for weeks now Ive told my nurses but nothing was addressed so I blew up at the resident cause enough is enough so they called 911 I refused to go they tried to convince me at the end of it all they put me on a watch they called me the aggressor when all this time Ive just let it go so that there wasn’t any problems I would definitely take your loved ones somewhere else ⸻

DO NOT send your family member here. They are incompetent and RUDE on top of it. The supervising nurse on the floor AUTUMN should be fired. She was dismissive when concerns were brought to her. Questioning why my mom didn't just go to a different rehab if we had all these issues. The nurse was disrespectful when my questioned what meds she was being given. When i asked AUTUMN what was the protocol, she goes "some do and some don't" WHAT?? There are 4 patients to a bathroom which is shared through an adjoining door between the rooms. That was not explained so to my mothers surprise when she opened the door SOMEONE IS SITTING ON THE TOILET. Back to AUTUMN she said well your mom opened the door on the other patient. My question WHY wasn't the door locked? Are all the bathrooms like that? SHE DOESNT KNOW.... MAAAMMMM You're a supervisor and you don't know?? I spoke to Victoria the Director. Nice lady that tried to fix the issues. moved my mom from a shared room to her own. In the transfer an employee STOLE HER 10ft CHARGER! Like what tacky nonsense is this. Victoria had another one purchased the next day. Must have come from Temu because IT STOPPED WORKING a week later. My mom is on oxygen the nurse did know how long a tank would last being that her concentrator was NOT WORKING. Her Cardiologist would call with her numbers regarding the pressure on her heart. The next day i called to check in with the nurse on duty, HE KNEW NOTHING ABOUT IT. Sooo she could have had a medical emergency overnight. PT was decent. After i spoke to them regarding giving her weights and other items to use without sanitizing them first. Especially when the flu was at an all-time high. My mom had to carry on about them coming into her room without a mask. back to AUTUMN, she told my mom that she was a troublemaker. Mom threatened to call 911 for a wellness check. Told the CNAs to not go into her room. The most helpful person there was Tina!! She looked out for my mom; made sure she was comfortable in a bad situation. Patients up and down the hallway, the worker yelling down the hall, talking and laughing all loud at all hours of the night. When the director is on property it is totally different than at night or on the weekend. No way to contact a nurse for an update, no consistency on the front desk answering the phone. When they transfer you back, NO ONE ANSWERS, you end up leaving a message and you MIGHT get a call back. There was blood on the floor, and the cleaner cleaned it with water and a PAPER TOWEL with his foot. IS THAT HOW BLOOD IS HANDLED??? Told them to PACK UP MY MOM before we have to tear down that place. They are just interested in you staying the 14 days so that THEY CAN GET THE INSURANCE PAYMENT. The place is not well kept and the food is DISGUSTING!! Mind you from her lunch, she didn't drink her ginger ale, she wanted to take them home. the man packing up her room said it was the property of the facility. HER INSURANCE PAID FOR IT. DON'T DO IT TO YOURSELF. This place is TRASH and needs to be CLOSED.

The rehab staff was very professional and patient with my mother. They made my mom feel like she was the only one there. They were super sweet all the time. They stayed busy with all the patients but always made everyone feel special. I can say I miss that staff. Great job keep up the good work.

My loved one is currently living at Swan Lake. Staff is above and beyond. Shout out to Stuart for his amazing and prompt attention to concerns. Susan in the business office takes time to review any issues and is more than patient & pleasant! The receptionists are super friendly and always greet guests with a nice, warm smile. Second floor staff including, head nurse Hillary always there to answer any questions. Friendly, caring & compassionate to their patients. Kenny from maintenance, fixed Bill’s tv within 5 minutes of reporting the issue. Extremely happy with the experience and peace of mind.

THIS PLACE IS A HEART WARMING PLACE TO BE.I WAS THEIR 3 MONTHS WITH IN THOSE 3 MONTHS I WAS THEIR TO GET BETTER I ALMOST DIED AND WITH ALL THE CARE AND LOVE I RECEVIED WITH THE NURSES AND PT .I STILL HAVE A LONG WAY TO GO .BUT I WOULD LOVE TO TAKE THIS TIME TO THANK YOU ALL .PAULINE WAS ON THE SCENE THANKS TO CHARMAIN HAVING TO RUSH ME TO THE HOSPITAL WHERE MY SON WAS PRONCED DEAD WITH IN 3 HR OF US GETTING THEIR.PAULINE NEVER LEFT MY SIDE THAT WOMEN NEEDS A MEDAL AND YOU AS WELL CHARMAIN THANK YOU BOTH.THEN CANT FOGET MY LOVE GABBY GARCIA WONDERFUL HEART I USED TO CALL HER A DAUGHTER I WISH WERE MINE MAD LOVE FOR YOU IN MY HEART THENTHEIR WAS SUEANNE ALWAYS A BEAUITFUL SMILE ALWAYS FULL OF LOVE .CANT FORGET AMONIE SHE WAS A LOVE TO ME MOSTLY WHEN PIZZA GOT DILVERED LOL .THEN TANISHA OMG WHAT A HEART OF JOY TO BE AROUND I CALLED HER MY DRUG LADY LMAO .
