I have to give a star in order for a review but this place really deserves 0 stars!! I have seen animals in the zoo and shelters have better treatment than the people there! Why does every room have an AC unit and the building doesn’t have central air?! The whole building smells horrendous. How and why is this place even open?! Then I saw an active fly trap hanging from the ceiling with bugs on them! This place needs to be shut down immediately because humans shouldn’t be treated so poorly all because they don’t have better insurance!

My mother is currently in this center and needs to get moved asap. TERRIBLE facility. As everyone else said, it smells like piss, everything is dirty and the staff sucks. She has to wait at least 90 minutes for help every time she needs to go to the bathroom. She was given insulin without her blood glucose levels being checked first. Im truly scared for my moms safety. Also, can the aids stop yelling at each other? We can hear you arguing from her room. Zero professionalism at this place.

This place is worse than worse. For the price we have to pay the treatment is below the bar on basic human decency. These people had my grandpa in dirty used clothes that didn’t belong to him. They left him in an uncomfortable broke down wheelchair, the sheets were covered in crumbs and miscellaneous stains (yes the crumb were his but not the stains) that looked like they haven’t been changed in weeks. The food was worse than high school cafeteria food, these people in here are already sick why are we feeding them slop?! And don’t even get me started on the smell and the rooms are broke down…

They left him in an uncomfortable broke down wheelchair, the sheets were covered in crumbs and miscellaneous stains (yes the crumb were his but not the stains) that looked like they haven’t been changed in weeks. The food was worse than high school cafeteria food, these people in here are already sick why are we feeding them slop?! And don’t even get me started on the smell and the rooms are broke down a disgusting. The walls were chipping and had marks all over them, just over all felt and looked dirty. As soon as you step in that elevator you smell pee. It feels like they just clean the lobby so it appears nice! There is barely any nurses around when you need them. I have no comment about the nurses as I understand being understaffed. This is purely a management issue. Get people in there cleaning and caring for our loved ones!

This place is horrible, I would never recommend this place to anyone who is looking to place a love one...they did not take care of my father properly and they did not get him the treatment he needed for a UTI thwy took so long without having him treated that it turned toxic and unfortunately he pasted away the day after they finally had him transported to the hospital becuase of it. This place seriously needs to shut down ASAP. They haven't even took the time to call or email any of us in the family to pay there respects or send thier condolences for the passing of my father because of thier neglect. Ive also heard stories from other people that have had love one's passway while at this place...smh they should be ashamed.

My friend went to rehab at Rochester center on Beahan Street. I visited her 3 times from anywhere from 2-4 hours each visit. I observed several health department violations: 1. Staff not wearing name badges 2. Never getting an answer when inquired who was aide assigned to my friend 3. Rehab and skilled nursing home residents are mostly in dining room/community room and not one person checking on any of them entire time I was there 4. Staff leaving residents to their own devices when meals come. They don't check to see if anyone needs help or even if they eat 5. When they moved my friend from recliner to wheelchair to wheel her to the table to eat, she did not have any pants on under her blanket, just a brief. 6. When my friend's family asked to have her taken to bathroom per my friend's request, they were told that they do not have an order for her to use bathroom, so she had to go in her brief!! This was after I had helped an aide put her on toilet 2 other days I was there. 7. Nobody toiletting anyone every 2 hours 8. Not one staff member was documenting any care they did give, at least not at "point of care" 9 Asked which days were friend's shower days and was told they couldn't find book with that information. 10. Staff nowhere to be found or seen most of the time. 11. On a weekend I was there, a housekeeper had headset on and was screaming at whoever he was talking to on phone as he pushed cart around. Never saw him do anything but push cart around and yell into phone 12. Too much turnover with staff. One nurse I talked to just started, couple aides were new, discovered social worker just started in October and Administrator there just 2-3 months 13. My friend was left in a brief for very long periods of time after bowel movement. 14. Friend also left in recliner in dining room most of the time, even thru the night. Family told by staff they have to keep her in chair because she's at risk for falls. They hardly let her rest in her bed! 15. The ONE bright point was that PT and OT were very good at job, very friendly, very nice and actually had name badges on! 16. My friend is no longer there because family had her removed. After one week there with all the nonsense, her mental and physical health were in serious decline. Oh, I didn't mention that the first time she was admitted, she fell on her face within 24 hours of being there and ended up at Strong Hospital for 3-4 days before she was readmitted back to facility. That lasted just over a week because family friend waited for days for someone to call her back to discuss the lack of care witnessed by several family members and friends. Come on, Department of Health....this facility has so many violations, I'm surprised you have let them continue to operate. If you don't believe me, go visit the place and by the way.....DONT MAKE AN APPOINTMENT TO INSPECT......JUST SHOW UP

My mother is currently a patient at this facility following a recent fall, and our family has been extremely concerned with the level of care and communication provided. At this time, there appears to be a significant breakdown in care coordination and responsiveness. The Center for Urology has made one documented attempt to contact the assigned social worker, Jennifer Bataglia, regarding medically necessary follow-up care, including coordination of a biopsy and/or discharge planning. This call has gone unreturned, which raises serious concerns about continuity of care and appropriate clinical oversight. Communication overall has been notably deficient. Our family has had to take on responsibilities that should be managed by the facility. Most concerning, my sister has personally had to assist our mother with bathing, as she has not been offered or assisted with a shower in approximately three weeks. This lack of basic hygiene support is unacceptable in a rehabilitation setting and calls into question adherence to standard care protocols. We were also informed directly by our mother that she tested positive for COVID, and our family did not receive any communication from the facility regarding this diagnosis. This lack of notification is concerning given the importance of timely updates on significant changes in a patient’s condition. It is also unclear whether appropriate isolation protocols and room placement are being followed. It is also unclear whether appropriate coordination is taking place between the facility’s medical team and my mother’s primary care physician and specialists. Given the need for follow-up care and discharge planning, clear and timely communication among providers is essential. At present, that level of coordination has not been evident. Additionally, there are ongoing environmental concerns. While the room itself appears to be maintained in terms of linens and routine cleaning, it is frequently uncomfortably warm and often has a lingering odor. The overall atmosphere can feel unsettling and does not reflect the level of comfort and care one would expect for a patient in recovery. Additionally, we observed concerning staff behavior during a recent visit. Two staff members were smoking in the vestibule area between the interior doors rather than in a designated outdoor space. This raises concerns regarding air quality and adherence to facility policies. There have also been instances of staff being noticeably loud and unprofessional in patient areas, which contributes to an unsettling environment for patients and families. Beyond comfort, hygiene, environmental conditions, and consistent care are critical components of recovery, dignity, and preparation for a safe and successful discharge home. The absence of consistent support in these areas is both alarming and concerning from a care planning standpoint. We are documenting this experience in hopes that it prompts immediate attention and corrective action from facility leadership. Our expectation is clear: timely communication, appropriate care coordination with outside providers, and consistent delivery of fundamental patient care services. At present, our experience has fallen well below those expectations. Update as of the following day, March 25th, 2026: The Department of Health has been contacted by my sister via phone and online submission and my other sister has contacted Jennifer Battaglia the social worker for immediate safe discharge plan to be coordinated for tomorrow. Jennifer has not returned my sisters call placed on Monday still. This is the second call now placed with no action on Rochester Center's end. Also, my sister replied to the generic email response left to my review and still no calls to any of us.

Please whatever you do avoid the Rochester Center. This is the worst facility in Rochester. The PT equipment needs to be updated. The staff is rude and poorly trained. If you come to this location you will not receive the level of service needed to heal.

Wish I could give no stars. Place is disgusting.
