I give this establishment a 2 star one for the shift supervisors which helped when I needed them & one for the few that really cared .. the day shift is fine once you get to the second and third shift it’s as if no one on my mothers unit cared and never answered her bell she had to call me each time she needed to go to the restroom . My mom scored high on her evaluation but at the same time was treated like a nobody . They rather put a diaper on you then to toilet the resident . When they did finally come in her room they checked her diaper and said it’s not wet . They wanted her to pee in…

Her both legs were swollen I called the nurse manager no answer no call back just an aid answering the phone to say I’ll have the nurse call and slam the phone down her legs were not like that when she was admitted… Very non mannered aids to say the least . I had to call 911 myself to have her sent out to hospital very concerned about her swollen legs they changed her medication I asked the nurse about it when I was on the unit and said they will call the dr never a call back . When sent to the hospital they ran test and blood work all came back normal while in the hospital she filled a large container from her curead I showed my mother and she said that’s all that she was holding in simply cause they pick and choose when you take a resident to the bathroom by the time we got her to hospital & home then the next morning her swelling was completely diminished & is in great spirts happy to be home and walking on her own . I do not recommend this facility if your looking for peace of mind how your loved ones are treated very unprofessional at every level . The facility puts on a good show at making the first floor look good clean and new once you get to the upper units it’s old and not well kept at all not a setting for any long term nursing care . Not to mention the food was horrible uncooked hard vegetables and what appeared to be the same dinner just a different main meal which was not any better the same hard vegetables daily . The unit kitchenette was filthy with trays from past days from the rooms not to mention all ice machines on units were broken each time I filled my moms I had to go to the 4th floor to get ice the only one working on 4 floors .
My grandmother was a resident here for rehab. When she first arrived things were a little rocky (due to being short staffed). But the rehab team did an excellent job with getting my mom better. She enjoyed the activities and on the days she felt down, she told me that Talya (the concierge) came in with a smile on her face and it brightened up her day. Kudos to the staff
The administration doesn’t answer phone calls, nurses are rude and don’t know what they are doing. Case manager acts like she’s untitled to speak and treat residents like they don’t have a voice. No with this momma bear. I reported her and the facility to the board in Albany, ny.
I recently had to stay here for rehabilitation after being in the hospital. The care team on the 3rd floor was absolutely Great. I was treated with such respect. I also want to say thank you to the rehabilitation team for getting me back on my feet. May God Bless you all. Thank you and you deserve 5 stars.
Workers can't drive. Plate ending in 2802 almost hit me because they were speeding to get infront of me..
I'm now at The Grand Rehabilitation and Nursing at River Valley. I don't like it. I'm healthy now; I got to get out of here. There are no activities. There's nothing new. I have a roommate, and I can't talk too loud now because he yells. We don't go out at all. They don't have a courtyard.
DO NOT PUT YOUR LOVED ONE HERE!!! INCOMPETENT, NEGLECTFUL, DISGRACEFUL, FILTHY, FOOD IS GARBAGE, AND BILL FOR SERVICES NOT GIVEN. ONLY A HANDFUL OF STAFF ACTUALLY CARE.
Did not offer to shower me for 5 weeks Food was inedible I lost 35 pounds
