I am so grateful for the Gowanda Nursing Home and Rehabilitation Center. They are giving excellent care for my Father. The Staff are providing excellent care and they are kind and courteous and responsive to my Father’s needs. I greatly appreciate the care my family has received at this facility.

Gowanda rehabilitation and nursing center was a very positive experience for my family and most importantly my father when he was a resident there for over a year. The staff were great from the friendly receptionist to the Drs and nurses and aides.. Amy who handles the financial services was so helpful and knowledgeable she helped make a very difficult transition much easier and answered all my questions and concerns. Bonnie is very compassionate and helpful she helped us in finding an assisted living facility for my farther. I would recommend this facility for anyone that has a loved one in…

Gowanda rehabilitation and nursing center was a very positive experience for my family and most importantly my father when he was a resident there for over a year. The staff were great from the friendly receptionist to the Drs and nurses and aides.. Amy who handles the financial services was so helpful and knowledgeable she helped make a very difficult transition much easier and answered all my questions and concerns. Without assertive outside intervention, residents are subject to a persistent pattern of clinical neglect and administrative silence. I. Without assertive outside intervention, residents are subject to a persistent pattern of clinical neglect and administrative silence. I. UNAUTHORIZED PROCEDURES & CLINICAL NEGLECT (Violation: 42 CFR § 483.24) For six months, I have endured an unmonitored protrusion at my stoma site. This culminated in October 2025 with an unauthorized medical procedure where a nurse engaged in "fishing" for a metallic object in my abdomen without a physician’s order or a sterile field, resulting in a physical rupture. "THE SILENCE OF LEADERSHIP: The most damning evidence of the lack of accountability here is the administrative response—or lack thereof. When I reported the unauthorized invasive 'fishing' procedure and the coerced medication to PA Madonna, the facility took zero action. No investigation was launched by the Director of Nursing, no statement was taken by the Administrator, and no reprimands were issued. In this building, reporting a violation doesn't lead to a solution; it leads to medical gaslighting and silence." II. MEDICAL GASLIGHTING & THE EROSION OF AUTONOMY Despite nursing confirmation of a foreign object, PA Madonna has practiced a campaign of medical gaslighting, dismissing physical evidence. Most disturbingly, the facility disregards the Federal Right to Refuse Medication (42 CFR § 483.10). I was coerced into taking medication under duress; in this institution, administrative convenience takes precedence over bodily a III. 61 DAYS OF HYGIENIC ABANDONMENT I have been deprived of a basic human shower for exactly 61 consecutive days. The justification of a "malfunctioning shower chair" is an inadmissible excuse for two months of hygienic neglect, violating fundamental federal sanitation standards. IV. PREVENTABLE PHYSICAL TRAUMA & SHEARING INJURIES I have suffered three separate skin tears due to systematic negligence during bed transfers. These injuries were the direct result of the improper manual removal of the Hoyer pad while I was in bed—a failure to follow safe handling protocols designed to prevent shearing forces on the skin. Additionally, I have been left in my own urine for four hours. Most egregiously, a respected Seneca Elder was physically assaulted; the administration merely relocated the perpetrator rather than terminating their employment. V. THE NORTH SHORE VANGUARD: A TRUSTED COMPASS To families: your loved ones are only in capable hands if you secure the care of Haley, Piper, Tony, Cheyenne, or Ty. My 2-star rating is a direct reflection of their individual integrity. VI. ADMINISTRATIVE SILENCE & REGULATORY FAILURE I submitted formal documentation of these grievances to the Administrator on Jan 12, 2026. Under federal law, they are required to provide a written resolution. Instead, they have chosen 36 days of total silence. VII. THE PHILOSOPHY OF THE PAPER TRAIL I publish this audit as a matter of civic duty. While individual experiences vary, institutional accountability is only achieved when survivors document the truth. I am laying this "paper trail" so that when the administration eventually falls flat on its face due to these systemic failures, there will be a permanent record of the warnings they ignored. VIII. A PLEA FOR SENECA NATION LEADERSHIP As a citizen of the Seneca Nation, I am calling upon our leaders. Our elders deserve protection, not silence. It is time to protect our own. I was pleasantly surprised when entering this location how inviting the atmosphere was. I have been to other nursing homes and this place made me feel welcomed from the first moment. Walking the halls for a visit, I noticed the cleanliness and kind staff. Everyone was busy but still made it a point to smile or say hi.

I had my grandpa there and the way they treated him was so spectacular and wonderful. Everyone was so loving and caring.

As a daughter, trusting others with your parent’s care is never easy—but from the moment my father arrived here, I knew he was in the right place. The staff treated him with genuine kindness, patience, and respect, and that meant everything to our family. Every nurse, aide, and therapist went above and beyond to make sure he felt safe, comfortable, and encouraged during his recovery. Communication was excellent, and I always felt informed and welcomed whenever I had questions or concerns. Thanks to the wonderful care he received, my father has now been discharged home, stronger and more confident than when he arrived. I am incredibly grateful for the compassion and dedication shown to him every single day. This facility truly feels like a place where people care—not just about patients, but about families too. Thank you from the bottom of my heart. 💛

We are incredibly grateful for the care and compassion shown to our loved one at this nursing home. The staff treats residents with dignity, patience, and genuine kindness. From the nurses to the support staff, everyone goes above and beyond to make residents feel safe and cared for. The facility is clean, welcoming, and truly feels like a community. We couldn’t have asked for better care.
