Brook at High Falls is a state of the art facility, anyone that needs good care and a beautiful place to come for rehab it's definitely the #1 place to be. They recently renovated and the place is like a 5 star hotel. The staff is also so caring and loving. We had a great experience there!!!

Went to visit a friend and the place need repairs in the building itself. Also my friends room was not clean. Water cups all over the floor, food stacked on his nightstand. Clothes and towels on the chairs. We had nowhere to sit. Someone needs to hang things up in his closet.

My father came here for physical rehabilitation. There were ants and other bugs in his room. He could not walk, and relied entirely on the staff. He sat in his own feces and urine for hours at a time. 8 hours at times ! PT/OT only spent 30 minutes collectively with him a day. That’s 15 min from a physical therapist and 15 minutes from an occupational therapist, working with a human being who can not walk, who use to walk, in hopes of regaining muscle strength to be able to walk again. (Walker reliant ) PT walked in his room and said to him I only have you for 15 minutes. They would roll him…

He felt like he couldn’t call for help, ask for water anything. Absolutely terrible experience. There is also no PT/OT on the weekend.

Well my complaint is that the place is very under staffed and ones that are there are over worked. There are residents that need more attention then others and that are lacking the care they need.im not blaming aides im blaming that they should have more help so someone isn't waiting to be changed as there sitting in wet or soiled diapers or bedding..they shouldn't have to sit in there room or bed because there is anyone to help.people are falling because no one comes fast enough when they push button for help because there isn't enough help they are attending others or on break. There should at least be 3 aides on at all times.not have 1 aide doing most of work.one more complant as of today finding out my moms bed was taken because she was taken to hospital as she fell and on observation at hospital. No phone calls stating that happened and still has stuff there and was told no bed available from hospital she is at.i am beyond angry and disgusted that they would do something like that.i also called left message no call back from this place.my mom has lots of things there. So all her rehabilitation will go down tbe drain because of this..now my moms rehab will go down the drain they declined her coming back just for getting blood transfusion but they could not do until couple days after hemoglobin wasnt as low then for hospital to do.so we have to pay 500 dollars a day to hold a bed which is rediclious. Why would a rehab nursing do that to someone. Thats pretty rediclious apparently its all about the money and not the patients care..a person should not have to pay that much for a bed hold. They pay enough with insurance and co-pays. Maybe 100 a day would be more responsible.then no one there to realy talk to .I made a call earlier. If this place truly cared about there patients they would not do that. Then get brushed off very angry..but apparently no one gives a dam.now my mom has to wait to go somewhere else to start over.even the doctor at hospital said my mom should be sent to an out patient place for transfusion not to emergency every time and put my mom in distress. Like set up witha hemotoligist. My mom has gone through a lot..and still is.truly should be ashamed to do this to an elderly person that has been there awhile and put into distress and having to find placement somewhere else and farther away..very uncaring facility

My mother was placed in the hospital because her hemoglobin Levels Were on the low side. She was brought to emergency under the Advice of the staff on duty at the time later come to find out The doctor question why she was brought to emergency given that she has Alzheimer Dementia, she was put into Distress We were then told by the doctor she should’ve been monitored at the nursing home and only brought to the hospital when and if it came to the point that she needed to have a transfusion, And only brought to the hospital as an outpatient. Having her stay in the hospital only Worsened her condition. The therapist staff that was working with her Helped a lot She was able to say more words because she lost her ability to speak after having a stroke. She was eating on her own and no longer having to eat puréed food. Although some of the staff worked with her well, especially one of the nurses, but not all of them were on the same page in giving her the Attention that she needed . Often Soiling herself because of the lack of attention. I feel like they were Licking in the right amount of staff and sometimes over tired because they have more than one job also viewed excessive phone use instead of giving attention to their patients or residence. I also feel that the facility could be cleaner. There’s so much more I can say regarding the condition of this particular nursing home/therapy facility. I’m just going to close with saying think twice search well before leaving your loved ones in any nursing facility

My best friend's grandma was treated extremely poorly. The facility is dirty, the staff are negligent, and the food is awful. Giving a patient who is to stay away from sugar a donut, piece of pie, and apple juice is beyond me! This place needs to be shut down or needs new management. They also refused to call 911 when her grandma needed care. Luckily, my friend pushed and pushed to get her grandma to the hospital to get the care she needed. This place needs to be reported for negligence to their patients, and people need to be fined and held accountable. ZERO STARS.

My grandmother was placed at The Brook at High Falls after a stroke, and her care there was unacceptable and unsafe. She is diabetic with a history of blood clots and lymphedema. Despite this, she was repeatedly served donuts, juice, pie, pasta, and other high-sugar/high-carb foods. Staff gave her large doses of insulin without clearly documenting or explaining it, and her blood sugar spiked as high as 351. She complained of severe pain in her left leg and feet for hours, but staff only gave her Tylenol and did not provide appropriate lymphedema care (no compression wraps, no thorough exam of her feet). We had to call 911 ourselves after the facility initially refused to call EMS or allow transfer to the hospital. Only after persistent calls did EMS finally come and take her to Strong Memorial Hospital. The environment was also unclean and uncomfortable, with strong odors and poor monitoring. Staff could not even tell me what she had eaten for meals, which is critical for a diabetic patient. This is not just poor service — it is neglect. Families should be aware before placing a loved one here. We are filing formal complaints with the NY Department of Health.

As someone who has worked in healthcare for years across multiple facilities in the area, this is by far the worst I’ve encountered. We got my loved one out of there as soon as possible. It’s unclear whether this place is being shut down or undergoing a remodel, but it looks nothing like the photos. On my first visit, I walked into what looked like a dingy, makeshift office. The next day, that same room was completely empty except for some leftover trash. The entire front half of the building looks abandoned as there are many dingy, empty spaces/rooms. Basic supplies are also lacking. There was no hand soap in the bathrooms, and no hand sanitizer dispensers anywhere. I am curious how the workers are managing to maintain adequate hand washing. They also are short on linens. They were making wash cloths by cutting up towels and reusing them multiple times. The level of care was also problematic. They claim to do rounds every 2 hours, but I stayed for longer than that without seeing a single staff member check on my family member or their roommate. My family member was denied bathroom help because “it wasn’t time yet,” leading to accidents. They also will put you into bed soiled because "it's not time for rounds, it hasn't been 2 hours". Unsurprisingly, this lack of care led to bedsores. Although this is supposed to be a rehab facility, there isn't an actual physical therapy room. My relatives PT consisted of walking from the bed to the bathroom and back. It could very well be that one of the empty rooms used to be a PT space, but why send patients here for rehab if you know the building isn't currently equipped for it? There also seemed to be a lot of issues with the staff but I won't harp on that as I can imagine working in a place like this with little supplies can be very frustrating. You can only due so much in a place that seems to be barely holding it together. The state really needs to show up for a surprise inspection.
